New Driver Packet
New DOT drivers must complete a new Union Pacific Driver’s Packet. All forms must be completed, signed, dated, and faxed to 402-233-3388. Include a enlarged (200x size) copy of your current driver’s license and medical card to ensure accurate and timely processing.
To verify if a fax has been received and processed, check with your immediate supervisor.
