New Driver Packet

New DOT drivers must complete a new Union Pacific Driver’s Packet. All forms must be completed, signed, dated, and faxed to 402-233-3388. Include a enlarged (200x size) copy of your current driver’s license and medical card to ensure accurate and timely processing.

To access new driver packet or specific forms, you must access the Union Pacific internal home screen.

To verify if a fax has been received and processed, check with your immediate supervisor.
