The Well-Being Assessment is a confidential health questionnaire that assesses lifestyle and overall health. It provides a better understanding of the key factors that affect overall health and well-being, and offers ways to improve. It is brief, voluntary and is divided into six domains including: emotional well-being, physical well-being, healthy behaviors, work environment, basic access, and overall life evaluation.
Completing the online Well-Being Assessment provides automatic access to a wellness portal, featuring a well-being plan customized from the Well-Being Assessment to fit personalized needs. Among the specific health information, it provides valuable resources such as:

- sample grocery lists
- exercise articles
- significance of health screening numbers
